Closing the Health Inequalities Gap: An International Perspective
Abstract
This report presents an analysis of official documents on government policies to tackle inequalities in health from 13 developed countries. All countries recognize that health inequalities are caused by adverse socioeconomic and environmental circumstances. However they differ in their definitions of inequalities and in their approaches to tackling the problem. Sweden and Northern Ireland have structured their overall public health policy to tackle the underlying determinants of inequalities in health. England is the only country with a separate comprehensive policy. Most countries also have policies on poverty, social inclusion, and social justice. These are motivated by a concern for human rights and dignity and deal primarily with the underlying causes of health inequalities. While broadly setting the same overarching goal, policies on health inequalities show many different features. Policymakers face two challenges: to ensure that strategies to tackle the macroenvironmental factors feature in policy on inequalities in health, and to ensure that health becomes a prominent issue in social justice policy. Few countries have a coordinated approach to tackling inequalities in health.

Health Consequences of Poverty for Children
This document summarises the physical, emotional and psychological health consequences of poverty for children. Maternal and child health are intimately linked and, consistent with the inter-generational approach, advocated by the Acheson Report. I start with a brief review of the effects of poverty and low socio-economic status on maternal health - particularly as it affects readiness for pregnancy and foetal well-being. Birth weight has major consequences for survival in early infancy, health throughout childhood and into adult life and the impact of poverty on this is considered next. Death, disability and illness in infancy and childhood. closely linked to birth weight and poverty, are discussed before examining the consequences of poverty for the emotional and psychological health and well being of children. Poverty and material deprivation in rich nations appear to have a negative effect on parenting, leading among other things to child poverty, parenting and child protection are reviewed. Finally, the links between poverty, educational attainment and children's health are discussed.

Huge poor–rich inequalities in maternity care: an international comparative study of maternity and child care in developing countries
This paper describes poor–rich inequalities in use of professional delivery and antenatal care for 45 developing countries and compares these to inequalities in use of child health services. By presenting various aspects of inequalities in the use of maternity care, and by contrasting these to inequalities in the use of child health care, this paper seeks possible explanations for the inequalities observed in maternity care.

Income inequality and health status: a nursing issue
Abstract
Objective
To review the association between income inequality and health status, and consider an appropriate nursing response.
Primary Argument
Nursing has a rich heritage of advocating for a healthy society established on a foundation of social justice. The future legitimacy and success of public health nursing depends on recognising and appropriately addressing the social, economic and political determinants of health in the populations served. There is an incontrovertible association between population health status, absolute income levels and income inequality. Thus, along with other social determinants of health, income differentials within populations must be a fundamental consideration when planning and delivering nursing services. Ensuring that federal and state health policy explicitly addresses this key issue remains an important challenge for the nursing profession, the public health system and the Australian community.
Conclusions
Higher mortality and worse health status occur in societies with higher income inequality. The relationship between income inequality and health appears to be determined both by relative access to resources for health gain and relative social position. The association between greater income equality and improved health may be explained by improved social cohesion. As social factors are at the root of much of health inequality, this knowledge needs to invoke political action and advocacy from the nursing profession to promote the development of healthy public policy. Including indicators of income inequality when planning and monitoring nursing services will enable services to measure to what extent they are based on the principle of social justice.

Abstract
The global burden of neuropsychiatry diseases and related mental health conditions is enormous, underappreciated and under resourced, particularly in the developing nations. The absence of adequate and quality mental health infrastructure and workforce is increasingly recognized. The ethical implications of inequalities in mental health for people and nations are profound and must be addressed in efforts to fulfil key bioethics principles of medicine and public health: respect for individuals, justice, beneficence, and non-malfeasance. Stigma and discrimination against people living with mental disorders affects their education, employment, access to care and hampers their capacity to contribute to society. Mental health well-being is closely associated to several Millennium Development Goals and economic development sectors including education, labour force participation, and productivity. Limited access to mental health care increases patient and family suffering. Unmet mental health needs have a negative effect on poverty reduction initiatives and economic development. Untreated mental conditions contribute to economic loss because they increase school and work absenteeism and dropout rates, healthcare expenditure, and unemployment. Addressing unmet mental health needs will require development of better mental health infrastructure and workforce and overall integration of mental and physical health services with primary care, especially in the developing nations.

The NHS Plan: Improving health and reducing inequality
No injustice is greater than the inequalities in health which scar our nation. The life expectancy of a boy born into the bottom social class is over nine years less than a boy born into the most affluent social class. The gap between health needs and health services remains stubbornly wide.
The worst health problems in our country will not be tackled without dealing with their fundamental causes. This means tackling disadvantage in all its forms - poverty, lack of educational attainment, unemployment, discrimination and social exclusion. It means recognising the specific health needs of different groups, including people with disabilities and minority ethnic groups. Improving health is now a key priority for all government departments. Action will be taken to step up the cross-governmental focus on health and inequalities.
The NHS too has a stronger role to play in prevention, as well as working in partnership with other agencies to tackle the causes of ill health so as to reduce health inequalities.
