Root Cause Analysis is a retrospective review of a problem undertaken in order to identify what, how, and why it occurred. The analysis is used to identify areas for change, make recommendations and implement sustainable solutions, to minimise risk or prevent re-occurrence in the future. This method is for organisational use rather than individual reflection.
