This audit tool has been developed on behalf of the Health and Social Care Information Centre. The NHS Health Check programme aims to prevent heart disease, stroke, diabetes, dementia and kidney disease in patients between the ages of 40 and 74 in England.
This tool enables the practice to review if any patients between the ages of 40 and 74 (without any of ten specific diagnosis codes or a prescription for statins within the last 12 months) need to be recalled for an NHS Health Check appointment. It also incorporates an inbuilt mail merge for recall letters.
This new audit tool is available to all PRIMIS Hub members, basic and full. However, we are planning to release a second version, which will report on patients outcomes, available only for full members.More details about the audit tool are available here.
