Tina tries to work around Adam's shifts and Megan's need for care. She works as a cleaner for private customers who are understanding of her need to work flexible hours. She has just found out that she is 12 weeks pregnant. This was not planned. She thought it was the menopause.
Adam was diagnosed with type 2 diabetes 4 months ago and is trying to juggle 2 jobs having just been made redundant from a 3rd job. He does shift work at a local power station and also works some shifts at a taxi firm.
Megan is 17 years old and was diagnosed with Global Developmental Delay. There is no known cause for her delayed development, but it affects all areas, including communication. She attends a special needs school and attends after-school provision each day. She is a regular visitor to hospital and Megan's hospital consultant has indicated that she needs to transfer to adult hospital services. A keyworker has been appointed to work with Megan and her family to ensure that this transition is as smooth and uneventful as possible. Megan has artificial feeds via a gastrostomy (PEG) through her stomach wall multiple times each day and overnight. The routine at home has evolved to incorporate Megan's need for positioning and re-positioning on a regular basis in addition to her nutritional needs. She has an adapted bedroom and bathroom downstairs with a hoist. Her wheelchair, feeding tubes and boxes take up alot of space.
Sam is 13 years old and is an anxious boy who gets picked on at school. As a result he has a low self-esteem and recently has appeared very withdrawn and frightened of going out and of going to school. At home, he spends most of his time playing online games and looking at social media and videos. School report that he spends most of his breaks alone and his attainment has fallen off very rapidly from what they would have expected. Sam has cuts on his arms and there is a smell of cannabis in the home.
