Chronic Wound Assessment
Observe, document, record
When assessing a wound you need to observe, document and record:
How did the wound occur?
- As a result of trauma? If so how? Could
it have been prevented?
- Is it a pressure related wound?
- If it is a human or animal bite/injury
or there is a possibility of a foreign object in the wound further advice
will be needed.
How long has it been present?
- If the wound has not healed is there an underlying health problem preventing healing?
