Eye Opening - Spontaneous
This patient's eyes are open spontaneously. This would score 4.
Eye Opening - To sound
This patient's eyes open to the voice of the nurse. This would score 3. Please remember that patients may be asleep or have hearing impairments which may require a louder voice.
Eye Opening - To pressure
This patient does not open their eyes to speech and the nurse administers the trapezius squeeze as a central painful stimulus. Other forms of painful stimuli such as sternal rub or supra orbital pressure should not be used due to bruising, enduring pain and the risk of further injury. This would score 2.
Eye Opening - No response
This patient does not open their eyes to speech and the nurse administers the trapezius squeeze as a central painful stimulus. The patient still does not open their eyes. This would score 1.
