Report an issue
You can use this form to inform the School of any behaviour or circumstance that you believe is making somebody feel uncomfortable, excluded or threatened, particularly on the basis of their race, disability, belief system, gender-related issues or sexual orientation. You can also use this form to list any suggestions you have for improving things within the School.
- All contributions are anonymous unless you choose to leave your name.
- If you are leaving an anonymous message, please include a memorable word (please do not use one of your passwords) which you can use to contact us and withdraw your data if necessary.
- Please enter your email address if you want a reply.
- If your concern is such that we need to investigate a particular individual, we may very occasionally need to ask you to contact a member of staff in person to confirm details.

You can also, or alternatively, contact your line manager, your Head of Division, Head of School, or a member of the Dignity Advisor Network.
