5. Preparing the dressing change
The nurse is now preparing for conducting the dressing change. Prior to undertaking the dressing she asks the patient several key questions that enables the nurse to build a holistc assessment of the patients care requirements.

Joy: I'm sleeping in here and living in here.
Nurse: OK.
Joy: Because i can't get up those stairs
Nurse: I'm just going to pop my equipment on here Joy, if that is OK?
Joy: Yes, yes, that is alright.
Nurse: Would you like me to sit over here?
Joy: Yes, good.
Nurse: Lovely! Lovely! So you have been in hospital recently then?
Joy: Yes, yes I erm - I hurt my leg.
Nurse: Right, OK, OK. So you have got a wound on your leg which as you know is the purpose of my visit today.
Joy: ...and it is a bit painful.
Nurse: OK, ok - So you were saying that you are sleeping on the setee?
Joy: Yes, yes. I can't get up the stairs.
Nurse: Right OK, and is that a new thing. Could you get up the stairs before?
Joy: With difficulty, I could, but now with this leg I'm nervous of hurting it anyway.
Nurse: OK, OK, so because it is painful you are finding it difficult getting upstairs.
Joy: Yes, it is very painful!
Nurse: OK, so what about in terms of tablets, what are you taking for pain at the moment?
Joy: Ah, well that is fine. I have got this marvelous pack, everything comes packed up in little dosettes.
Nurse: OK.
Joy: Ah - I get one in the morning and get one in the evening, and providing I get the right day everthing goes fine.
Nurse: Great, they are very good in terms of helping people to remember to take their medication and things - aren't they?
Joy: Absolutely - yes!
Nurse: But.. because you are saying that your leg is quite painful, we might just need to have a little chat about whether you need to have a look at your pain relief, you know, and have that reviewed. So we will have a talk about that in a minute, if that is OK?
Joy: Very good. Excellent!
Nurse: I just need to go and wash my hands, if that is alright?
Joy: Yes, yes!
Nurse: Where abouts would you like me to go?
Joy: Well there is a downstairs toilet and cloakroom there and i think there is some of that stuff to keep...
Nurse: Lovely!
Joy: ...Sanitiser.
Nurse: Sanitiser, that is brilliant!
Joy: Hope you manage?
Nurse: Yes, oh thankyou! I will be back in a minute...
Joy: Put the light on.
